Medication Assistance at BHA

BHA will help you get your medications free of charge, or at a reduced cost, directly from the pharmeceutical companies that manufacture the medications. Please download and review these forms for more information about the medication assistance program and how to apply.

To Become a Patient at BHA

Patients are seen by appointment.  Spanish interpreters may be available to those who do not speak English, but it is recommended that you bring an interpreter with you if you have one available.
Charges are calculated from the Federal Poverty Scale and Medicare Fee Base.
Patients are required to bring:

  • A photo identification
  • AND at least one of the following:
  • Proof of income
  • W-2's
  • Three current paycheck stubs
  • Unemployment letter from unemployment office
  • Letters from employers or
  • Government benefits letter
  • Last Federal Tax Return

Patients residing in rehabilitation centers or shelters must provide a dated letter (for the day of service) confirming their residency and employment status.
